FormOS is an online form builder that allows users to create forms, collect submissions, upload files, generate documents, and integrate with external storage and website platforms.

This Privacy Policy explains how FormOS collects, uses, stores, protects, and shares information when you use our website, dashboard, public forms, integrations, plugins, and related services.

By using FormOS, you agree to the collection and use of information described in this Privacy Policy.

1. Information We Collect

FormOS may collect the following types of information.

Account Information

When you create or manage a FormOS account, we may collect:

• Email address

• Password authentication data

• Business or workspace name

• Billing or subscription information

• Account preferences

• Login and authentication activity

Passwords are stored using secure hashing. We do not store your plain text password.

Form and Workspace Information

When you use FormOS to create forms, we may collect and store:

Submission Information

When someone submits a form created through FormOS, we may collect:

The form owner controls the forms they create and is responsible for ensuring their forms comply with applicable privacy and legal requirements.

Uploaded Files

FormOS forms may include file upload fields. Uploaded files may be stored using connected third-party storage integrations selected by the form owner, such as Google Drive or Dropbox.

FormOS may store limited metadata about uploaded files, including:

• File name

• File type

• File size

• Storage provider file ID or reference

• File URL or reference

• Upload time

• Form ID

• Submission ID

• Upload status

Payment and Billing Information

If you subscribe to a paid plan, payment processing may be handled by a third-party payment provider such as Stripe. FormOS does not store full credit card numbers on its servers.

We may store subscription status, plan details, billing customer IDs, invoices, payment status, and related billing metadata.

Technical and Usage Information

We may collect technical information such as:

• IP address

• Browser type

• Device type

• Referring URL

3. Google User Data and Google Drive Integration

FormOS allows users to connect their Google account so that uploaded form files and generated documents can be saved directly into the user’s own Google Drive.

Google Data We Access

When a user connects Google Drive to FormOS, we may access the following Google user data, depending on the permissions granted:

• Basic Google account information, such as the connected Google account email address or profile identifier, used to show connection status and identify the connected Google Drive account.

• Google Drive folder information selected or created by the user for storing FormOS files.

• Google Drive file and folder metadata required to create folders, upload files, organize submissions, and display upload status.

• Files uploaded through FormOS forms that the form owner chooses to store in their connected Google Drive.

• Generated FormOS documents, such as completed PDFs, when they are saved or sent as part of a form workflow.

How We Use Google User Data

FormOS uses Google user data only to provide and operate the Google Drive storage integration. This includes:

• Connecting a user’s Google Drive account to FormOS.

• Allowing the user to choose or configure a Google Drive storage folder.

• Creating organized folders for forms and submissions.

• Uploading respondent files into the connected Google Drive folder.

• Saving generated documents related to form submissions.

• Displaying integration status, upload status, and limited file metadata inside the FormOS dashboard.

• Troubleshooting Google Drive upload issues when support is requested.

What We Do Not Do With Google User Data

FormOS does not sell Google user data.

FormOS does not use Google user data for advertising.

FormOS does not use Google user data to train artificial intelligence models.

FormOS does not transfer Google user data to third parties except where necessary to provide the requested FormOS service, comply with applicable law, or protect the security and integrity of the platform.

FormOS does not expose Google Drive OAuth tokens to public form submitters.

Storage and Retention of Google User Data

Files uploaded through FormOS are stored in the connected user’s Google Drive account when Google Drive storage is enabled. FormOS may store limited metadata about uploaded files, such as file name, Google Drive file ID, file reference or URL, upload time, form ID, submission ID, and upload status, so that the form owner can view and manage submission records.

FormOS stores OAuth connection data and integration credentials securely on the server side to keep the Google Drive integration working. These credentials are not shown to public form submitters.

Users may disconnect Google Drive from FormOS at any time. Disconnecting Google Drive stops future uploads to that Google Drive account through FormOS. Files already stored in the user’s Google Drive remain under the user’s control and can be managed or deleted directly from Google Drive.

User Control and Revocation

Users can disconnect Google Drive from FormOS inside their FormOS account settings. Users can also revoke FormOS access through their Google Account permissions page.

After access is revoked, FormOS will no longer be able to upload files to that Google Drive account unless the user reconnects it.

Protection of Google User Data

FormOS protects connected storage data through authenticated dashboard access, ownership checks, server-side integration handling, and safeguards designed to prevent public exposure of storage provider tokens.

Public form submitters cannot choose the form owner’s Google Drive destination and cannot access the form owner’s Google Drive credentials.

4. Dropbox and Other Storage Integrations

FormOS may also allow users to connect storage providers such as Dropbox.

When a user connects a storage provider, FormOS may access account, folder, file, and upload metadata needed to provide the integration. FormOS uses this information only to upload, organize, display, and manage files according to the user’s selected settings.

Users may disconnect integrations at any time. Files already stored in the user’s external storage account remain under the user’s control.

5. Website, WordPress, and Shopify Integrations

FormOS may provide embed tools, WordPress plugins, Shopify apps, or theme app extensions that allow users to display FormOS forms on external websites.

When a FormOS form is embedded on another website, FormOS may process:

• Form display requests

• Form submissions

• Uploaded files

• Signature or initials data

• Browser and technical metadata

• Embed source metadata where available

Form submissions are processed by FormOS and are not stored inside WordPress or Shopify unless a user separately configures another integration.

6. Cookies and Similar Technologies

FormOS may use cookies or similar technologies to:

• Keep users signed in

• Protect user sessions

• Remember preferences

• Improve security

• Analyze usage and performance

• Support billing, authentication, and integrations

You can control cookies through your browser settings, but disabling cookies may affect FormOS functionality.

Form owners may delete forms and submissions inside FormOS where deletion features are available. Some metadata, audit logs, billing records, or backup records may be retained for a limited period for security, legal, operational, or compliance purposes.

Files stored in a connected third-party storage account, such as Google Drive, remain under the control of that storage account owner.

9. Data Security

We use reasonable technical, administrative, and organizational safeguards designed to protect information processed by FormOS.

These safeguards may include:

• Authenticated account access

• Password hashing

• Server-side OAuth handling

• Ownership and permission checks

• Secure handling of integration credentials

• Secure payment processing through third-party providers

• Access controls for dashboard and admin areas

No online service can guarantee absolute security. Users are responsible for using strong passwords, protecting their account access, and configuring forms appropriately.
